Ubertor has just launched a custom forms module that allows you to create what ever type of form you’d like and have it stored in your leads section of your control panel and send you an email. This new feature requires knowledge of HTML and we’re available to install it.
Here are a few screenshots of how we’ve implemented a “Newsletter Signup” custom form in the sidebar of our test website.

Finally! We use WuFoo and found it to always break from time to time, so this will be a better solution for sure.
